You may now renew your license online. Before you begin this process, it is important that you note the following:
You will need to submit a secure and verifiable document along with a notarized citizenship affidavit. Once you answer the renewal questions, you will be taken to a page that summarizes your answers for final review. At the bottom of this page there is a link that will take you to a subsequent page on which you may download both documents. This link is found right above the link for payment. If you do not choose to upload your documents during the renewal process, you may email or fax them directly to the address/fax number on the affidavit; however, please note that submission via email or fax WILL delay your processing significantly. Once you submit your credit card payment, you will not be able to upload the documents. Also, Dentists and Dental Hygienists are required to complete and submit a survey, the link for which is found on your receipt page.
